How does work flow in this system?

The board runs on two inseparable tracks. The preventive track: scheduled inspection rounds enter as cards in “Round”, where the inspector documents observations with photos and locations, and every violation becomes a task on the card. The reactive track: any employee reporting a hazard or incident enters the “Report” column immediately with its location and severity. Serious reports move to “Investigation”, where a team is formed to gather evidence and testimony and determine the root cause, not the surface one; then to “Action” to execute corrective and preventive measures, each with an owner and date. Lessons learned become “Training” cards targeting the groups concerned; the month’s indicators are summarized in “Report Summary”; and long-term actions stay in “Follow-Up” until their effect is proven.

Chat is the urgent-report channel: an imminent hazard is reported there instantly and converted into a card in the same moment, and coordination of rounds and drill times passes through it. Announcements are safety’s official voice: seasonal alerts (heat waves, hazardous maintenance works), drill results, and notices of updated safety instructions — whatever is published here binds everyone.

Dues tracks safety money: spending on personal protective equipment per site, safety-violation penalties on contractors recorded and collected, and the cost of incident remedies documented per card — so management knows the true cost of a single incident, a number that justifies preventive investment.

The safety officer reviews “Report” daily so no report sleeps unclassified; inspectors conduct rounds and document with photos; and action owners at the sites close “Action” with proof of execution. The board’s indicators — number of reports, action closure time, repeat violations at one site — are what is presented to management monthly from “Report Summary”.
